What is Ankylosing Spondylitis?
Spondylitis literally means inflammation of the bony building blocks of the spine (vertebrae) as the result of a disease process. Past spine injury (often several years before), severe arthritis, and past spine surgery can give rise to this debilitating condition.
Ankylosing Spondylitis (AS) is a chronic inflammatory condition that primarily affects the spine and sacroiliac joints, which connect the lower spine to the pelvis. Over time, this inflammation may lead to stiffness, pain, and in some cases, the fusion of spinal vertebrae, reducing mobility and flexibility.
AS is part of a group of diseases known as seronegative spondyloarthropathies, but now it is commonly referred as spondyloarthritis, and it can also affect other joints, tendons, ligaments, and even organs such as the eyes and heart.
The exact cause of Ankylosing Spondylitis remains unknown but heritability is frequently considered as a significant contributor in the manifestation of the disease. This ailment shows strong association with major histocompatibility alleles particularly HLA-B27, which may account for up to one third of the genetic effect.
What are the symptoms of Ankylosing Spondylitis?
Signs and symptoms od Ankylosing Spondlylitis include:
- Low back pain, especially worse in the morning or after periods of inactivity.
- Bamboo spine/Poker back where the whole spine becomes a single rigid column.
- Eye Inflammation (Uveitis), causing pain, redness, and blurred vision in some individuals.
- Tenderness of affected vertebrae
- Lumbar movements limited and painful
- Pain worsening at night, stiffness more in the early morning, postural changes, dorsal kyphosis, spinal flexion limited
- Pain gets worse with repeated movements, such as lifting and bending
- Reduced chest expansion is an early feature.
- Generalised tiredness due to inflammation.
- Pain especially in shoulders, hips, or knees.
What are the causes of Ankylosing Spondylitis?
- Genetic Factors: A strong link exists with the HLA-B27 gene, which is found in many individuals with AS.
- Immune System Dysfunction: AS is considered an autoimmune condition where the immune system may attack healthy joint tissue.
- Environmental Triggers: Infections or other external factors may play a role in triggering symptoms in genetically predisposed individuals.
Risk factors
Several factors may increase the likelihood of developing AS:
- Past spinal injury or surgery
- Severe arthritis or trauma history
- Family history of AS or presence of the HLA-B27 gene.
- Most commonly begins in late adolescence to early adulthood (typically between ages 15–40).
- More common in men than in women, though women may experience milder symptoms.
Ayurvedic Perspective
Ankylosing Spondylitis is a vata-dominant sandhiroga with ama (accumulated toxin) association affecting the spinal region. The classical symptoms of as per Ayurveda include Angmarda (pain), Aruchi (anorexia), Aalasya (fatigue), Gaurav (stiffness), Jwara (fever), Apaka (indigestion), Trishna (thirst), Angashoonta (oedema). Again, if there is the involvement of upadhatus in the form of nerves emerging from the spinal region, there will be pain radiating from the lower limb as similar to sciatica. If there is more vata aggravation, the spine will assume the shape of bamboo with limited or absent chala guna of vata (spinal mobility)
