What is Sciatica ?

Sciatica is a painful  condition characterized by pain radiating along the sciatic nerve, often originating in the lower back and traveling down one or both legs. It can result from various causes, including herniated discs, spinal stenosis, or even pregnancy. The pain may be sharp, burning, or tingling and can be debilitating.

What are the symptoms of Sciatica ?

  • Pain radiating down the back of your legs starting from the buttock region
  • Numbness in the affected legs especially in the calf and toes
  • Tingling or pins and needles sensation along the course of the nerve
  • Difficulty walking or standing for long periods
  • Mild ache to a sharp, burning sensation or excruciating pain.
  • Muscle weakness in the affected leg or foot.
  • Pain may worsen with prolonged sitting, standing up, coughing, or sneezing.
  • Symptoms are more likely to manifest on one side of the body.
  • In severe cases, burning sensation.
  • Weakness and numbness in lower limb.
  • After standing in place or while walking, feeling of heaviness in leg and inability to move leg.
  • Shooting pain in the lower limb that makes it difficult to stand up.
  • Inability to stand straight.
  • Constant pain sensations in the lower limb even while sleeping.

What are the causes of Sciatica ?

  • Herniated Disc: The most frequent cause, where the soft center of a spinal disc pushes out and presses on the nerve root.
  • Spinal Canal Stenosis: Narrowing of the spinal canal causes pressure on nerves, resulting in inflammation
  • Piriformis Syndrome: Muscle strain or spasm or tightness of the piriformis muscle in the buttock, causes irritation of the nearby sciatic nerve.
  • Spondylolisthesis: One vertebra slips forward over another, narrowing the nerve exit opening.
  • Degenerative disc disease: This causes breakdown of spinal discs with age.
  • Trauma: Injury to the spine or sciatic nerve or even fracture of the pelvis, or hip joint could lead to sciatica
  • Obesity: Excess weight gain can put pressure on the spinal disc causing disc compression.
  • Underlying diseases: Endocrine disorders such as diabetes mellitus
  • Strenuous activity: Activities such as lifting weights or or prolonged sitting or standing and over-exposure to cold wind
  • Genetics: Hereditary factors also pave the way for neurological disorders

Risk factors

  • Ayurveda views sciatica as an imbalance caused by Vata dosha alone or by both Vata and kapha dosha and is termed as “Gridhrasi”, where “ghridra” in sanskrit means vulture and the person affected by ghridrasi/sciatica walks like a vulture due to the affliction of sciatic nerve, which leads to stiffness, radiating pain, restricted movement, and difficulty in walking. This condition is characterized by pain originating in the lower back or hip region and radiating down the leg along the path of the sciatic nerve.

Here the aggravated Vata dosha, when combined with Ama (toxins) or sometimes with Kapha (leading to stiffness and blockage), obstructs the nerve pathways in the lower part of the body. This vitiated Vata lodges in the Kati (lower back/hip) region and travels down the leg along the sciatic nerve pathway, causing characteristic symptoms like Shula (pain), Stambha (stiffness), Toda (pricking sensation), and impaired movement.

Lifestyle Recommendations for Sciatica

Ayurvedic treatment for sciatica includes vata and kapha balancing panchakarma and herbal remedies. It helps bring the vitiated ‘dosha’ back to the state of equilibrium and thereby establish the state of health. The aim of the treatment is to balance the vitiated doshas particularly vata through various oleating and steaming therapies. The following therapies are beneficial in sciatica and can be personalised as per individuals need.

Panchakarma Therapies:

Vasti (Medicated Enema):

Considered the most effective treatment for Vata disorders. Specific types like Kashaya Vasti (decoction) and Anuvasana Vasti (oil) cleanse the colon (a primary site of Vata) and nourish the system, directly relieving nerve irritation.

Abhyanga (Therapeutic Oil Massage):

Using specific Vata-pacifying oils to relax muscles, improve circulation, and soothe nerves.

Swedana (Herbal Steam Therapy):

Helps to liquefy toxins, reduce stiffness, and alleviate pain. Often follows Abhyanga.

Patra Pinda Sweda (Elakizhi):

Fomentation using boluses filled with medicinal leaves fried in oil, highly effective for pain, inflammation, and stiffness.

Kati Basti:

A specialized treatment where warm medicated oil is retained over the lower back region, relieving pain, lubricating joints, and nourishing tissues locally.

Along with these therapies, Agnikarma, Sira Vyadana (Venesection) are very effective in treating Sciatica.

External Applications

Local application of medicated oils and herbal pastes (Lepa) to reduce pain and inflammation.

Internal Medications

Prescribing specific herbal decoctions (Kashayam), tablets (Vati/Gulika), powders (Choornam), and medicated ghees (Ghrita) containing herbs known for their anti-inflammatory, analgesic, nerve-tonifying, and Vata-balancing properties (e.g., Guggulu formulations, Ashwagandha, Rasna, Dashamoola, Sahacharadi).
